At Forever Yours Tattoo Company, we take compliance with Florida laws and statutes very seriously. We require both the health department forms AND a consent form for client tracking for all minors receiving services.

 In Florida, written consent from a parent or guardian is required to tattoo a minor because it protects the minor and ensures compliance with health regulations. The form also protects the tattoo artist. 

Why is consent required? 

• Protects minors: Prevents minors from getting tattoos without permission by forging their parent's signature

• Protects tattoo artists: Ensures that tattoo artists have proper documentation of legal consent

• Complies with health regulations: Ensures that all parties involved adhere to legal frameworks

What's required for a minor to get a tattoo in Florida? 

• Be accompanied by a parent or legal guardian
• Provide government-issued photo identification
• Have the parent or legal guardian submit written consent on the state form DH 4146
• Have the parent or legal guardian provide proof of relationship

Penalties:

Violating these requirements is a second degree misdemeanor.

-Children 0-12 may receive ear piercing services ONLY.

-Children 13-18 may receive the following piercings: EARS, HELIX, NOSTRIL

-Children 14-18 may receive the following piercings: EARS, HELIX, NOSTRIL, TRAGUS, CONCH, ROOK, LIP, EYEBROW, NAVEL, SEPTUM (AT PIERCER AND PARENTS DISCRETION ONLY. NO MINOR WILL BE GIVEN A PIERCING THEY ARE NOT EDUCATED ON OR READY TO RECIEVE.)

Minors must also:

• Be accompanied by a parent or legal guardian
• Provide government-issued photo identification OR birth certificate
• Have the parent or legal guardian submit written consent on the state form DH 4146
• Have the consent forms notarized by a notary public

MINORS WILL NOT BE ABLE TO RECEIVE THE FOLLOWING PIERCINGS:

• Tongue/Bridge/Industrial/Dermals/Any Piercing that Requires Privacy.

We provide detailed instructions on how to take care of your tattoo or piercing to prevent infection and promote healing. We also sell aftercare products such as tattoo balm and sterile saline solution.
